The direct aortic cannulation for acute type A aortic dissection.
This article describes the surgical techniques demonstrated in our video, “The Direct Aortic Cannulation for Acute type A Aortic Dissection”. Acute type A aortic dissection is a life-threatening condition requiring emergency operation. The choice of arterial cannulation sites to institute cardiopulmonary bypass is still a topic of controversy. Central cannulation directly through the ascending ...

Results Patient characteristics such as preoperative complications and shock status were almost similar between the groups. Although intraoperative parameters such as total cardiopulmonary bypass time, hypothermic circulatory arrest time, minimum bladder temperature and percentage of total arch replacement were also similar between the groups, ascending aortic cannulation significantly reduced ...

Methods We enrolled consecutive 45 patients who underwent type A AAD repair from January 2007 to March 2012. Central cannulation was applied under ultrasound guidance using the Seldinger technique in 33 patients and in 12 patients through peripheral cannulation. After the distal aortic anastomosis, antegrade systemic reperfusion was established with prosthetic side arm. Two groups were compared...

We present a simple new method for establishing cardiopulmonary bypass to treat type A acute aortic dissection. Antegrade blood flow in the true lumen is theoretically best to prevent malperfusion and retrograde embolization. Central cannulation can be performed in the true lumen of the dissected ascending aorta using an ultrasonographically guided puncture technique such as 'Seldinger's method'.

OBJECTIVE Antegrade perfusion for type A acute aortic dissection prevents malperfusion and retrograde cerebral embolism during cardiopulmonary bypass. Prompt establishment of antegrade perfusion via ascending aorta may improve the surgical results of type A dissections, especially in the situations of hemodynamic instability. Thus, we evaluated the efficacy of use of the dissected ascending aor...
